Long-Term Resident Care

Hospice of New York offers a special program for residents of long-term care facilities (nursing homes) who are facing a life-limiting illness.

Working in partnership with the long-term care staff, the Hospice Team develops a joint plan of care tailored to the specific needs of the resident and his or her family members.

Hospice professionals provide expert assistance with pain and symptom management as well as spiritual and emotional support for patients and their loved ones.

As with any Hospice care, focus rests on creating an environment of understanding among patients, families and caregivers and enhancing the patient’s quality of life in every possible way.

Long-term care services complement and supplement the services already provided by the long-term care facility staff and may include:

  • Scheduled visits by experienced Registered Nurses to assist with pain control and symptom management
  • Coordination of medical equipment for comfort and symptom management in addition to the equipment routinely provided by the long-term care facility.
  • Supplemental personal care for residents provided by Certified Nurse Aides.
  • Psychosocial support for residents, families and the long-term care staff.Spiritual support, which may include clergy as requested, for residents and families.
  • Companionship and emotional support provided by trained volunteers.
  • Consultation and emergency care available from Hospice professionals 24 hours a day, every day of the year.
  • Bereavement counseling and support for family members throughout the resident’s illness and up to a year or more, as needed, after death.

Eligibility for Hospice Long-Term Care Services

There are times when home is not the preferred option for a patient facing a terminal illness. Sometimes a patient needs more care than can be managed at home or symptoms require more intense management than a family caregiver can provide. Other times a patient's support system breaks down due to illness or accident, or friends and family who simply need a temporary respite from caregiving. For these instances, Hospice of New York has Hospice Inpatient Units located conveniently throughout its service area.
  • Hospice Inpatient Unit provides services in partnership with the facility
  • Admissions are coordinated with the patient's doctor and the Hospice team
  • Well-coordinated patient care smoothes transfers to and from the unit and facilitates management of medications between home and inpatient settings
  • Personalized care based on in-depth knowledge of the patient and family's medical history.Inpatient services are reimbursed by Medicare, NYS Medicaid and many private insurers and HMOs once specific clinical criteria are met.
  • Inpatient services are typically short-term. A stay in the inpatient unit is intended to stabilize symptoms that cannot effectively be managed in the homecare setting.
  • Services and room assignments are made without regard to race, color, creed, age, religion or national origin.
The Hospice Team will coordinate whatever individualized services or therapies.
