The Hospice Team

The hospice team will coordinate and provide any individualized services or therapies needed, when they are needed most. Once you choose hospice, the hospice team has been engaged, you, our patients and families, are never alone. Hospice Nurses provide emergency on-call assistance 24-hours a day, every day of the year.

In addition to the disciplines listed below, the Administrator and Director of Clinical Services ensure quality, well trained and responsive staff.

  1. Patient’s Physician works with the hospice nurse to design and oversee a plan of care.
  2. Hospice Physician provides consultation and overall medical guidance for the hospice team and participates in the patient/family care planning. Yes, our physicians make house calls.
  3. Ancillary Therapists provide additional types of therapy such as physical, occupational, speech and respiratory, when medically indicated. Many of our patients enjoy the services of our Music Therapist.
  4. Counselorswhich may include clergy, offer emotional and spiritual support. Our Bereavement Counselors provide services to family members for 13 months following the loss of your loved one. 
  5. Registered Nurses: A Registered Nurse and the Patient Care Coordinator (PCC) is responsible for coordinating all clinical services you will receive. A community Hospice Nurse assigned to you will provide skilled pain management services and caregiver education to help reduce pain, avoid complications for the patient and enhance family support. They coordinate the provision of medical equipment, supplies and medications. Your Community Hospice Nurse will communicate with the attending physician, consult with the hospice physician and is “on call” 24 hours a day, seven days a week for patients and families.
  6. Social Workers assist families in identifying their personal and community resources and developing support networks to aid in coping with a life-limiting illness.
  7. Certified Nurse Aides / Home Health Aides, under the supervision of the Hospice Nurse, provide patients with personal care and assistance with the activities of daily living.
  8. Trained Volunteers offer assistance with the activities of daily living and friendly support, companionship and encouragement to patients and family members.
