Care at Home:
While making no attempt to hasten death or prolong life, hospice provides the services and support that can lessen the discomfort at the end of life. In order to receive hospice services, a patient must be admitted to the program with a certification provided by the physician, based on certain admission criteria.
A hospice nurse will evaluate your needs. In consultation with you, your  physician, our hospice physician and the Interdisciplinary Team (IDT)  will develop a Plan Of Care. A plan that is individually created based on symptoms, support system and preferences. A registered nurse will visit, in your home, along with a social worker and any other services your care requires . Your RN, along with her Team Clinical Assistant , under the supervision of your Patient Care Coordinator will determine your requirements for medical equipment, medication, and therapies that are related to your terminal illness.
Hospice services may include anything from skilled nursing care, pain management and symptom control to physical therapy, homemaker assistance and dietary or emotional counseling and more. Hospice care is delivered within a coordinated team approach and palliative treatment to reduce or eliminate symptoms and improve quality of life. Treatment modalities are determined in consultation with the attending physician.
