    Responsibilities: Experienced social worker needed to provide psychosocial assessment and supportive interventions of hospice patients and families in conjunction with the hospice interdisciplinary team. Services will be provided in SNFs, IPUs and homes in the Bronx

    Qualifications:
    • LMSW/LCSW in NY state
    • 2+ years of SW experience (healthcare) preferred.
    • Demonstrated experience in discharge planning.

  • Hospice Physician

    Hospice of New York is Seeking a Full-Time Hospice Physician with proven Hospice, Palliative, End- of-Life or Geriatric Care experience to provide medical direction for the Hospice team and Hospice In-Patient unit in Queens and Nassau Counties.

    Main responsibilities: The physician will function as an integral part of the interdisciplinary clinical team with the collaboration and under the supervision of the Medical Director to provide service to Hospice of New York patients. Will review and assess patients at admission for prognosis and for our plan of care for each patient.

    Qualifications:

    • Licensed to practice in NY State & DEA license.
    • Board Certified/Eligible in Hospice and Palliative Care, Internal Medicine, Family Practice or Geriatrics
    • Clinical expertise in caring for Hospice or Palliative patients.
    • Ability to work within a diverse interdisciplinary team.
    • Ability to work with a diverse patient population.
    • Experience with EMR (electronic medical records) documentation a plus.
    • Reliable motor vehicle.

  • Home Health Aids

    Seeking experienced HHAs to provide services to hospice patients in SNF, IPU and private residences in Brooklyn, Queens, Manhattan, The Bronx and Nassau County.

    Responsibilities: Provides personal care including bathing, dressing, grooming. Assists patients to ambulate. Assists patients with elimination up to use of external colostomy. Performs other simple procedures as an extension of therapeutic services in accordance with the hospice plan of care. Homecare or Hospice exp preferred

    Requirements:
    • HHA Certificate.
    • 2 + years of HHA experience;

  • Staff Development Coordinator –Fulltime/Part-time

    Seeking a strong hospice nurse and advocate for end-of- life care, with excellent communication skills and a desire to share knowledge and experiences with other healthcare professionals.

    Responsibilities: Main responsibilities to develop, coordinate and present training and development programs for all employees. Responsible to assess and identify companywide developmental needs to drive training initiatives. Identify and arrange suitable training solutions for employees to promote and maintain competencies. Will also be responsible for planning and coordinating training and development activities for our contracted facilities and contracted vendor staff. Position will be in Long Island City, Queens, NY

    Qualifications:
    • BSN minimum, MSN preferred
    • NYS RN License required.
    • 3 + years of Training & Development and leadership skills.
    • Exceptional written and oral communication skills.
    • Proficiency in Microsoft Word applications, Excel, PowerPoint Presentations and medical records applications.
    • CHPN certification required within 6 months of hire.
    • Prior experience in hospice healthcare setting a definite plus.

  • Hospice Nurse Practitioner

    Seeking 1 Full-Time Nurse Practitioner with, Palliative, End of Life Care or Geriatric Care experience to conduct Face-To-Face visit in Nassau County and Queens.

    Main responsibilities: Coordinates care for Hospice patients within the Hospice of New York service area. Functions as an integral part of the interdisciplinary team. Will also conduct patient recertification assessments in various hospice settings.

    Qualifications:
