  • Community Hospice RN - Bronx

    Seeking a Full-Time Community Hospice RN

    Provides nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary Plan of Care. Supervises Home Health Aides. Will provide end-of-life care in SNFs, IPUs and homes.

    • Registered Nurse licensed in NY state.
    • 2+ years of RN experience (med-surg/homecare/ hospice) preferred
    • Recent BSN graduates will be considered for our mentorship program
    • Bilingual English/Spanish preferred.
    • Computer proficiency.
    • Ability to work with limited supervision.
    • Strong interpersonal skills.
    • Collaborative and team-oriented personality.
    • Reliable motor vehicle required.

  • Team Clinical Assistant

    Supports the Patient Care Coordinator and the clinical team by providing admin support as well as scheduling HHAs and LPNs. Ordering DME supplies for delivery to patients and scheduling patient transportation. Maintains in office stock/inventory of medical supplies. Also responsible for maintaining patients medical records in accordance with regulatory standards and agency policy.

    • High School Diploma or GED required.
    • Previous admin experience ( healthcare preferred)
    • Demonstrates excellent verbal and written communication skills
    • Demonstrates organizational skills and knowledge of medical terminology.
    • Proficiency in Microsoft Word and Excel required.
    • Ability to multitask and meet deadlines.
    • Professional telephone etiquette required.
    • Exceptional interpersonal skills and ability to work as a team.
    • Certification in Medical Records Information Technology ( a plus)
    • Bilingual English/Spanish, English/Cantonese and or Mandarin ( a plus)
